Self Declaration for Trainee Teachers

You will find enclosed in your interview pack a General Teaching Council for England (GTC) Suitability Declaration form. To be considered for a place on a teacher training course, you must fully complete this form before coming to interview.

This form is a self-declaration by applicants, and does not replace the CRB checks which will be used to confirm that the information given is accurate.

Please note the following:

1. Liverpool John Moores University cannot offer a place on any of its teacher training degrees unless this form is completed by the applicant, and subsequently assessed and cleared by LJMU and/or the GTC.
2. If you answer YES to question 1 and you are currently wholly barred from working with children, please contact us urgently.
3. If you answer YES to any question on the form, please ensure that you elaborate on the reverse of the form. If further space is required, please attach additional sheets clearly stating your full name, date of birth and UCAS/GTTR application number beginning 10XXXXXXXX, and sign and date the sheet.
4. You are not expected to have a Teacher Reference Number at this point, so please leave this field blank. This form is for the purposes of Provisional Registration with the GTC; you will be fully registered on completion of your training, and once you have been awarded QTS.
5. Please do not send your Suitability Declaration back to us ahead of your interview; we will collect it from you at interview.
6. If your case is referred to the GTC for consideration, any offer cannot be made until we have confirmation of clearance from the GTC.
7. Please ensure that you sign and date the form appropriately.
8. If it is subsequently found that any information entered on this form is incorrect or incomplete, any place offered by the University may be withdrawn.

If your Circumstances Change

If you are made an offer following interview, and your circumstances in relation to the answers on your original GTC Declaration change - e.g. you are convicted of a crime, or cautioned - please complete the following steps:

1. Download, print and complete a new GTC Self Declaration Form here.
2. Send your completed GTC form to:
   • Recruitment Team
   • IM Marsh Campus
   • Aigburth
   • Liverpool
   • L17 6BD
3. Include a covering letter with your full name, date of birth, and your UCAS/GTTR ID number starting 10XXXXXXXX

It is vital that you inform us of any change in your circumstances as we will compare your CRB Form to your GTC Declaration following enrolment, and if there are discrepancies your offer may be withdrawn.
